چکیده

CARDIORENAL AXIS Normally, the cross-talk between the heart and the kidneys occurs through atrial-renal reflexes3, which contribute to maintaining the total body volume in the normal range. In a nonfailing heart, any increase in atrial pressure diminishes the arginine vasopressin release (AVP)4 through the Henry Gauer Reflex, decreases renal sympathetic tone5, and increases the atrial natriuretic peptide6. All of these increase the urinary sodium and water excretion rate. In HF, however, there is blunting of these reflexes in the low-pressure circulation, probably secondary to being overridden by reflexes initiated in the high-pressure arterial circulation.

The Dose-Dependent Effect of Nesiritide on Renal Function in Patients with Acute Decompensated Heart Failure: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

BACKGROUND Conflicting renal effects of nesiritide have been reported in patients with acute decompensated heart failure. To answer this controversy, we performed a meta-analysis of randomized controlled trials to evaluate the influence of nesiritide on renal function in patients with acute decompensated heart failure. METHODS Articles were obtained from PubMed, Medline, Cochrane Library and ...

Biomarkers in acute heart failure.

The care of patients with acutely decompensated heart failure is being reshaped by the availability and understanding of several novel and emerging heart failure biomarkers. The gold standard biomarkers in heart failure are B-type natriuretic peptide and N-terminal pro-B-type natriuretic peptide, which play an important role in the diagnosis, prognosis, and management of acute decompensated hea...

Reconsidering ultrafiltration in the acute cardiorenal syndrome.

Aggressive diuretic therapy in a patient who is hospitalized for acute decompensated heart failure often leads to progressive renal dysfunction despite persistent congestion. The underlying mechanisms of this so-called acute cardiorenal syndrome are complex and not fully understood.1,2 As initial therapy in this setting, ultrafiltration as compared with diuretic therapy may result in a higher r...

Erratum: Serum bilirubin as a prognostic marker in patients with acute decompensated heart failure

BACKGROUND/AIMS Several prognostic markers for heart failure (HF) have been determined but the importance of liver function tests (LFTs) remains unknown. The aim of this study was to determine the prognostic significance, if any, of abnormal LFTs in acute decompensated HF. METHODS All adult patients (> 18 years of age) who were admitted to a community hospital with a diagnosis of acute decomp...

Acute decompensated heart failure and the cardiorenal syndrome.

Heart failure is one of the leading causes of hospitalizations in the United States. Concomitant and significant renal dysfunction is common in patients with heart failure. Increasingly, the syndrome of heart failure is one of cardiorenal failure, in which concomitant cardiac and renal dysfunctions exist, with each accelerating the progression of the other. One fourth of patients hospitalized f...
